Liability Release Form

Release of All Claims 
In consideration for being accepted by City Takers, Inc. for participation in any:

(For Volunteers and Event Participants)

In consideration of being allowed to participate in any City Takers volunteer activities, events, or service projects, I acknowledge and agree to the following terms:

1. Release of Liability

I, being 18 years of age or older, agree to release, forever discharge, and hold harmless City Takers, Inc., including its directors, pastors, employees, agents, and volunteers, from any and all liability, claims, or demands for personal injury, illness, death, property damage, or expenses of any kind that may arise from my participation in any City Takers activities.

If I am signing on behalf of a minor participant, I also release and hold harmless City Takers, Inc. and all associated parties from liability related to that child’s participation in the same manner.

2. Assumption of Risk

I understand and accept the risks associated with volunteering, including physical activity, transportation, interaction with the public, and service-related tasks. I voluntarily assume full responsibility for any injury, illness, or loss that may result.

If signing for a minor, I acknowledge and accept these risks on their behalf as well.

3. Transportation & Food

I acknowledge that City Takers may provide transportation and food in connection with volunteer activities. I fully and voluntarily assume all risks related to the provision or use of such services.

4. Medical Release

In the event of a medical emergency, I authorize City Takers staff or volunteers to seek appropriate treatment for me. I understand that I am responsible for any medical expenses incurred, and that my personal insurance will serve as primary coverage.

If signing for a minor, I give permission for my child to receive medical treatment, including emergency surgery or hospitalization if necessary. I accept full financial responsibility for all related expenses.

5. Media Release

I authorize City Takers, Inc. to use photos or videos taken of me during events or activities in print and digital media. I waive any right to compensation or approval for such use.

If signing for a minor, I give permission for my child’s image to be used in the same manner.

6. Ongoing Agreement

I agree to inform City Takers of any changes to the information provided. This consent remains valid unless revoked in writing by me or, if applicable, by the parent or legal guardian of a minor participant.
